Police have apologised for telling a Derbyshire woman her husband had died - over the phone.

Tragic James Dean - who had a history of drug and alcohol abuse and suffered from mental health problems after being abused as a child - was found dead in his home on Woodfield Road, Pinxton, on August 1 last year.
