A pilot scheme aimed at keeping people out of hospital is to be rolled out across North Derbyshire.
The scheme is part of the 21st Century #JoinedUpCare project, a partnership between the eight care organisations serving North Derbyshire, and has changed how health and social care professionals work together to look after people at home.
Teams have been working closer than ever before to provide joined up care for frail and elderly people, with better support at home and in the community to prevent them needing emergency care in hospital or care homes.
Dr Ben Milton, a Youlgrave GP and chair of NHS North Derbyshire Clinical Commissioning Group, said: “Major changes are needed in the way in which local healthcare is provided.
“People are living longer and their health needs are becoming much more complex and if services are not changed the local NHS faces a funding gap in five years’ time of around £150m.
“Clearly, we need to do things differently and our commitment is to transform healthcare so that it is centred on the needs of the patient.”
